SUBJECT:	MEDICAL RECORDS

Last August, the federal bankruptcy judge appointed to the KPC Medical Management / Chaudhuri Medical Corporation bankruptcy cases, approved an agreement with Iron Mountain, a records management company, to take possession of all remaining KPC/Chaudhuri medical records. Iron Mountain will search for and retrieve medical records for a court approved fee of $15 per patient, payable to Iron Mountain Inc. by credit card, personal check, cashier’s check, or money order. Please do not send cash. You will have the option of picking up your records in Cerritos, California, or having them shipped for an additional $6.50. (Total cost for retrieval and shipping is $21.50.)

NOTICE: The Iron Mountain agreement will terminate on August 31, 2002, and all of the remaining medical records will be destroyed after that date to protect the patients' privacy and confidentiality rights.

To receive your records, please complete, sign and date the attached authorization releasing your medical records. A separate form is needed for each family member (make copies of form, if necessary). This new form is required by Iron Mountain, even if you previously submitted a similar form to KPC in the past. Iron Mountain's goal is to have your record ready for delivery or pick-up within one week of ordering. Return the form(s) and payment to:

When Iron Mountain receives your form and payment, they will process your request. After they have retrieved your records, they will send them to you at the address indicated on your release or notify you to make arrangements for pick up in Cerritos, California. They will advise you if they are unable to locate your records.
